Configure page missing in admin panel

Hello,

I've installed your plugin and am looking forward to using it on my site. However, I don't seem to have a "configure" page in my admin panel. Only "donors" and "options" (see attached screenshot). And when I try to test the form by entering information and clicking "proceed", nothing happens.

Any help would be appreciated! Thanks.

The configuration for the plugin is on the "Options" page, there are no other options. With regards to the form not working when you clicked proceed, please provide me a link to the page on which the plugin is operating so I can investigate what might be wrong.
